PERSONAL DATA (PRIVACY) ORDINANCE

NOTICE TO PATIENT

As your doctors/nurses/health care providers, we have to ask you to provide your personal data (including your ID Card No. & health information) and we may obtain from any appropriate third party your medical history and any relevant information for your health care purposes and/or generally for medical purposes and/or for the purposes stated below.

When you provide personal data to us, please make sure that the data is accurate and complete. If you do not provide us with the information required or if the information provided is inaccurate or incomplete, our ability to provide appropriate health care to you may be affected.

Please note and accept that your personal data (including your ID Card No. & health information) may be made available to:

• other doctors/health care providers who require it for purposes related to your health care; or
• the Court if ordered or subpoenaed by the Court, or to such other persons as we are required by law.

If you require access to and/or correction of your personal data, you may do so under the Personal Data (Privacy) Ordinance. Please contact the doctor during clinic hours.

個人資料（私隱）條例

病人須知

在向本診所／機構醫護人員提供個人資料之前，請閱讀本通知書。

作為你的醫生／護士／醫護人員，我們會請你提供你的個人資料（包括身分證號碼及健康資料），亦可能向任何適當有關人等查詢你的醫療記錄及任何有關資料，作為有關你的診治或護理之用或一般性的醫學用途或下列用途。

當你提供這些個人資料時，請確保這些資料準確及完整。如果你不向我們提供所需資料或你提供錯誤／不完整的資料，便會影響我們為你提供的診治或護理。

請注意及接受你的個人資料（包括身分證號碼及健康資料）可能被交予：

• 其他醫生／健康護理人員，作為有關你的診治或護理之用。
• 法庭（如被傳訊）或法律指定的其他人士。

如果你有需要根據《個人資料（私隱）條例》要求查閱和／或改正你的個人資料，請於診症時間內與醫生聯絡。
